Birth Certificates and Medical Records at Chilton Hospital

To obtain a copy of your birth certificate:

Chilton Hospital does not maintain or issue birth certificates. To obtain a copy of a birth certificate for an individual born at Chilton Hospital, please visit The Pequannock Township website, or for further information please call the Pequannock Township Division of Vital Records at 973-835-5700 ext.127.

To obtain a copy of your medical record:

As a patient at Chilton Hospital, you have a right to receive prompt access to the information in your medical record.  You also have a right to obtain copies of your medical record at a reasonable fee, within 30 days, after written request from the hospital.

2. All requests must have a signed authorization to release records.

3. There is a charge for records ($1 per page) unless records are being released for medical treatment to the treating physician or facility.
