Hospitals Collaborate to Assess Health Needs of Northwestern, Central NJ
Community Will Have Access to County Results

Through Hospitals Hospitals in northwestern and central New Jersey are releasing their community health needs assessments on ways to improve the health of the communities served. The reports offer insight based on publicly available data as well as information gathered as part of a public survey conducted by hospitals in 2012.

In an effort to better identify and address the region’s health care needs, eleven hospitals in northwestern and central New Jersey collaborated to survey the community. The Community Health Alliance of Northwestern-Central New Jersey (CHANCe NJ) consists of Atlantic Health System, Saint Clare’s Health System, Robert Wood Johnson University Hospital Rahway, Chilton Hospital and Trinitas Regional Medical Center.

The survey was conducted to identify the most important health issues in the region, as well as by county, using a scientific method to validate health indicators and to compare results. In addition to engaging the hospitals and health systems in the CHANCe NJ program, the program also engaged more than 150 community representatives from a broad spectrum of organizations who assisted in identifying and prioritizing health needs for a community benefit plan.

“The collaboration of organizations throughout the community to identify priority areas as well as locate evidence-based programs which address those needs will be a key part of the next phase of this project,” said Chilton Hospital President and CEO Deborah Zastocki, DNP, RN. “We believe that the community’s health will be positively affected through education and outreach efforts as a result of each of these organizations working together.”

Each hospital involved in the CHANCe NJ survey has created an action-based plan which will address health concerns in their local community which were found as a result of the survey. Hospitals which overlap communities and counties are collaborating as part of a community-wide effort to create health goals for the counties and areas they serve. In Morris and Sussex Counties, Atlantic Health, Saint Clare’s Health System and Chilton Hospital are working collaboratively to address potential areas of opportunity to increase the health of the communities within the counties they serve.

In Morris and Sussex Counties, unmet community health needs have been identified in the following areas:
- Physical Activity and Nutrition
- Access to Care
- Mental Health
- Substance Use and Abuse
- Cancer

“It’s going to take all of us to improve the health status of our region,” said Dr. Zastocki. “No one organization will have adequate resources or scope of understanding to solve all problems identified in these assessments. However, we see this plan as providing guidance around how we, along with other area organizations and agencies, can collaborate to make the best health care available to the members of our community.”

The data on community health disparities was collected in a 2012 telephone survey to thousands of randomly selected households, primarily located in Morris, Sussex, Union, Passaic and Middlesex counties, who were called to participate in an anonymous and confidential health assessment survey. Following the survey, into 2013, the alliance continues to collect data gleaned from focus groups and interviews with stakeholders of minority and underserved populations, as well as those affected by chronic illness. Surveyors inquired about a variety of health questions, including access to health care; health risk behaviors such as smoking, alcohol and drugs, exercise and physical activity habits; health conditions such diabetes, cardiovascular disease, or stroke; and awareness of hypertension and cholesterol.
