Welcome to MyChilton

One Patient, One Record, One Atlantic Health System

Through 2018, we are transitioning our hospitals and medical offices to a new electronic medical records system called Epic. After February 4, 2018, MyChilton will no longer receive information from your medical record. We encourage you to sign up for MyChart to continue to have online access to your Atlantic Health System experience.

  • Allergies, conditions, and immunizations
  • Active home medications
  • Chilton past visit history
  • Discharge instructions from previous visits
  • Update personal data, including contacts and demographic information
  • Request appointments for selected hospital departments

In addition to viewing your own Chilton Medical Center health information, caregivers and health care proxies with authorized consents can be given access to the records of individuals under their care.

Proxy Access

  • A proxy access form must be completed for:
    • EACH proxy that will have access to a patient's health records
    • Patients who are under the age of 18 (These patients will not be able to enroll in MyChilton. Parents and/or guardians must fill out a proxy access form to access their records)
    • Any patients who would like to provide someone else with access to their health records
