| HIPAA Statement According to HIPAA regulations a covered entity must "...inform the individual that protected health information may be used and disclosed to carry out treatment, payment or healthcare operations." Since Chilton Hospital is a covered entity and must obtain informed consent as a normal course of treatment, no additional consent from patients for the purposes of operations or payment would be necessary. Every employee and affiliate of Chilton Hospital will take all necessary steps to ensure that any Protected Health Information (PHI), which may be observed during the course of treatment or business is kept confidential. Any uses of patient data are for the purposes of performing treatment, operations, or obtaining payment as defined in the HIPAA regulations.
RepTrax Program The Reptrax Program is a vendor credentialing program that requires vendors to meet our credential criteria to do business with the hospital. The credential program addresses various criteria depending on what services or products the vendor provides to Chilton Hospital.
If a vendor wants to do business with the hospital, they must register with Reptrax and complete the credentialing documentation. The vendor can register by logging on to the Reptrax web site at www.reptrax.com. Reptrax also maintains a record of every vendor that logs into the Reptrax Kiosk, located in the main entrance of the hospital.
Business Associate Agreement *(PDF) All vendors doing buisness with Chilton Hospital will need to review and complete the Business Associate Agreeement form and return it to their representative at Chilton Hospital.
